Adding an external gearbox makes a belt-driven lathe act like a geared head lathe, which means you get consistent torque at all speeds.
You also don't have to mess around with belts, pulleys, and a belt tensioner in order to change speeds.
LMMasterMariner did this build for around $100. You might pay a little more, depending on where you source the gear blanks. It's still a huge savings over buying a geared head lathe.
His original build was for a 12-speed gearbox, and the discussion includes a video showing construction and operation.
His second generation build has 27 speeds, and he also included 24 downloadable plans files with it.
